Our Doctors

David A. Wolf, OD
Optometric Physician

Dr. Wolf received his Bachelor of Science from Pacific University in 1981 and his Doctor of Optometry in 1983 from Pacific University College of Optometry. After practicing in Eugene, Oregon for nearly 15 years, Dr. Wolf moved to Lake Oswego in January 1998 when he purchased Lake Oswego Vision Clinic.

Dr. Wolf is involved in professional organizations on both national and local levels. He is involved with the American Optometric Association and the Oregon Optometric Association. He is currently the Oregon state professional representative for VSP vision. He served as a past member of Medicare’s carrier advising committee, and was a former board member of Eye Health Partners, which negotiates and manages contracts for eye health services. Locally, he is a member of Lake Oswego Rotary Club and Lake Oswego Chamber of Commerce. With his wife Sara and son Andre he enjoys golf, travel and sports.

Rachel G. Horrocks, OD
Optometric Physician

Dr. Horrocks is originally from eastern Washington, and received her Bachelor of Science from Central Washington University in 2004. She spent six years working in the eye care field, which prompted her decision to pursue Optometry. She subsequently moved to Oregon and earned her Doctor of Optometry Degree from Pacific University College of Optometry in 2008.

While in Optometry school, she was a member of Beta Sigma Kappa honor society, as well as an active participant in student government and the ambassador program. She organized community outreaches and vision screenings in local elementary schools and homeless shelters in the Portland area.

Dr. Horrocks completed an externship at a LASIK surgery center in order to better serve her patients in the area of pre and post-operative LASIK care.

She enjoys spending time with her husband Chris, son Cade and her friends and family.

Meredith
Certified Optometric Assistant, Marketing Coordinator

Meredith has been at Lake Oswego Vision Clinic since Dr. Wolf first opened the office in 1997.

As a Certified Optometric Assistant, she does preliminary exam testing and enjoys working with patients. Meredith's expertise and personality make you feel comfortable and welcome.

Meredith enjoys her two grown children Cooper and Decker and her chocolate labrador Lily.

Daisy
Patient Coordinator

Daisy is the smile of Lake Oswego Vision Clinic, and she will welcome you to our office, either on the phone or at the front desk.

Daisy's experience with insurance billing allows her to help you get the most value from your insurance coverage.

Daisy and her husband Shawn enjoy raising their daughter Ainsley.

Kirrin
Office Assistant

Kirrin comes to us with 7 years of customer service experience. Along with Daisy, Kirrin works up at the front desk answering phones and greeting you as you walk through the door. In her spare time, Kirrin enjoys keeping active, going to the beach and spending time with friends and family.

Debbie
Optometric Assistant

Debbie has prior experience working in an Eye Surgery Center. She works closely with Meredith to pretest patients for Dr. Wolf and Dr. Horrocks.
She has recently moved to downtown Portland where she enjoys the culture, art and walking along the river.

Michelle
Eyewear Specialist/Optician

Michelle has over 25 years experience in the optical field and enjoys getting to know each patient to understand their visual needs and wants. She uses the most up-to-date products available to help you see your best in all facets of your life. When not working, Michelle leads a healthy and active lifestyle with her family, friends and her dog Haley.
